INSTRUCTIONS TO request YOUR progenabiome test KIT:
STEP 1: FIRST, DOWNLOAD AND FILL OUT THE TWO FORMS BELOW
STEP 2: EMAIL COMPLETED FORMS TO RAY@PROGENABIOME.COM
Be sure to include both completed forms, and Note: your kit(s) will be sent to the mailing address on the "Demographics packet"
STEP 3: PAYMENT
Your test kit will include a Floratest™ and nose swab. The Floratest™ fee is typically $870 per kit, which includes the cost of the kit, shipping, the gut flora genetic sequencing, and results. HOWEVER, due to the current state of emergency, Dr. Hazan is accepting donations in lieu of full payment.
AFTER RECEIVING YOUR KIT:
Please carefully follow the instructions inside your kit to collect and prepare your sample, and to sterilize and return your kit.

RESULTS
Dr. Hazan will contact you once your analysis is completed to discuss her preliminary findings. Findings are for research only, not to be used for diagnosis or treatment at this time. The research is currently being conducted with a planned 1,000 enrollees. At the conclusion of the study, a report will be completed. We hope to use this information to develop an assay which will help us develop a better understanding of the microbiome in an effort to potentially offer a precise treatment solutions in the future. In the event you cannot afford the cost of kits, we will put your name on a list till we acquire funds through donations, grants, or other means to cover your testing. I hope this is helpful.
